Do you accept my insurance?
For major PPO Plans we are considered an Out-of-Network (OON) Provider; almost all of our patients with PPO Plans have OON Benefits.
Currently, we are not contracted with and cannot accept any HMO or Medi-Cal Plans.

What should I bring to and expect on my first visit?
Please fill out all of the New Patient Intake Forms sent to you and sign them prior to your appointment.
A valid, government-issued photo ID is also required upon your arrival.
On your first visit, expect to spend at least 20-30 minutes discussing your health history and any urologic issues you’ve experienced or are currently experiencing.
How can I best prepare for my first visit?
Complete all Intake Forms to the best of your ability prior to your appointment, including important information regarding all of your:
- Current physicians and their specialties
- Complete health history
- Emergency contacts
- Allergies
- Current medications
Please bring a valid government-issued form of photo ID and plan on arriving 5 minutes early to allow for parking and getting to our office.
I don’t have access to a printer, how do I fill out the forms?
It is not necessary to print our New Patient Intake Forms—they can be completed, signed and submitted online with your digital signature.
Can I fill out the forms in your waiting room?
If you would prefer to fill out the forms by hand, we can send printed copies to your residence.
If you plan to fill the forms out in our waiting room, please plan on arriving at least 1 hour prior to your appointment to allow enough time to read and complete the forms.
I’m currently experiencing a Urologic Emergency—what do I do?
If you have suffered a catastrophic injury and/or are in extreme pain, call 911 or go to your nearest Emergency Room.
Ascent Urology is not equipped to treat grievous injuries or provide emergency medical care.
I recently received a Cancer diagnosis, what’s the earliest I can schedule an appointment?
Please call our office and inform our staff for earliest possible availability.
I’m currently experiencing Kidney Stones, what’s the earliest I can schedule an appointment?
Please call our office and inform our staff for earliest possible availability.
I recently noticed visible blood in my urine, what’s the earliest I can schedule an appointment?

Do you accept walk-in appointments?
We are unable to accommodate walk-in appointments.
Do you treat children/are you a Pediatric Urologist?
No, Ascent Urology is not a Pediatric Urology Practice. The minimum age for our patients is 16.
There are other practices that specialize in Pediatric Urology, and we can provide a referral.
When was Ascent Urology founded?
Ascent Urology first started treating patients in March of 2016.
What are your hours of operation?
We are open Monday – Friday, from 9:00 AM – 5:00 PM.
We are closed for all Federal Holidays.
