COVID-19 (SARS-CoV-2) continues to be a public health and safety threat.

We strongly encourage you to take advantage of the freely available vaccines, and to get vaccinated today if you have not already done so.

Per the Centers for Disease Control (CDC) and LA County Department of Health, you must wear a mask—regardless of your vaccination status—in all Healthcare Settings. This includes Hospitals, Long-Term Care facilities, and all Doctors’ offices.

Your mask must fully cover your mouth and nose to be effective at protecting yourself and others from the spread of this virus.

You must wear a mask when you enter our building, elevator and office and keep it on at all times.

Regardless of whether you are:

• fully vaccinated,
• in our Waiting Room, Rest Room or Exam Room,
• alone or with the doctor,

your mask must stay on.

When you make an appointment with Ascent Urology you are agreeing to follow these guidelines.

If you are unable to comply with these guidelines for medical reasons, a Telemedicine appointment may be a solution.

A refusal or failure to follow these guidelines may result in a permanent dismissal from Ascent Urology.
